If Yes…
1) If you have a PPO insurance call your referring doctor today and ask them to write you a referral script to TERRIO.
2) You can pick the referral up from you referring doctor and call our Intake Coordinators at (661) 617-1182 to schedule your evaluation today!
3) You may also have your referring doctor fax your referral script to our Intake Coordinators at (661) 617-1187 and we will call you to schedule your first appointment!
4) If you have an HMO insurance call your insurance today and ask them to change your authorization to TERRIO. Once they have changed your authorization to TERRIO call our Intake Coordinators at (661) 617-1182 to schedule your first appointment!
If No…
1) If you have a PPO insurance call your referring doctor today and ask them to write you a referral script to TERRIO.
2) You can pick the referral up from your referring doctor and call our Intake Coordinators at (661) 617-1182 to schedule your evaluation today!
3) You may also have your referring doctor fax your referral script to our Intake Coordinators at (661) 617-1187 and we will call you to schedule your first appointment!
4) If you have an HMO insurance ask your referring doctor to submit your authorization for therapy to TERRIO. Once approved call our Intake Coordinators at (661) 617-1182 to schedule your first appointment!
SCHEDULING APPOINTMENTS
Appointments by Phone:
You may call our Intake Coordinators (661) 617-1182 to set up your first appointment. Please be ready to provide the following information: your personal data (name, address, phone number, DOB, etc.); insurance information (name of insurance, address, phone number, policy/group number, and primary subscriber name and DOB); employer information (company name and phone number), and referring physician information (name and phone number). If your insurance requires an authorization please have your authorization/claim number ready to give over the phone. If your insurance does not require authorization please make sure you have a current (dated within the last 30 days) referral script signed and dated from your referring doctor.
Appointments by Fax:
Click here to download the three forms and fax them to our Intake Coordinators at (661) 617-1187 with a copy of your insurance card (front and back) and current (dated within the last 30 days) referral script signed and dated from your referring doctor. Our Intake Coordinators will call you within an hour of receiving. You may also have your referring doctor fax over your referral script or authorization with your contact information.
Appointments by E-mail:
You may e-mail our Intake Coordinators at intakecoordinators@terriotherapy.com to schedule your initial evaluation. Please provide your name and contact number. This e-mail may only be used for initial evaluations.
